Diabetes Self-Management Education (DSME): One-on-one or group consultation with a pharmacist/certified diabetes educator to increase your knowledge of diabetes and the Seven Self-Care Behaviors used to prevent the progression of diabetes. (Individual $75, Group Session $50)
Medication Therapy Management (MTM): One-on-one consultation with a Certified Diabetes Educator (CDE) and Board Certified Geriatric Pharmacist and (CGP), promoting the safe and effective use of medications to help patients achieve the targeted outcomes from medication therapy. ($75)
Bone Density: Women over 30 and men over 45 should have a baseline bone density measurement. The test is quick, painless and produces a full report almost instantly. ($25)
Cholesterol Screenings: Our screening gives a breakdown of your HDL (good cholesterol), LDL (bad cholesterol), and Triglyceride Levels. Using these numbers along with some additional information, we can determine your risk for heart disease. ($30)
Body Fat Composition: Begin your fitness/ healthy lifestyle program with a resolution to lose body fat not simply body weight. Get a baseline body composition measurement in a full color, four page report. ($20)
Liver Function: Did you know that certain medications, including some for cholesterol and diabetes, can adversely affect your liver? A routine AST/ALT screening keeps you informed of your risk of potential liver damage. ($25)
HbA1c: This is the gold-standard indicator of diabetes control. HbA1c (glycosylated hemoglobin) values indicate the average blood glucose level over the past 2-3 months. Each reduction of 1% results in a 35% reduction in the risk of serious diabetesrelated complications. ($25)
Insulin Pump Training: A Certified Insulin Pump Trainer (CPT) is available to train patients on the correct use and maintenance of insulin pumps. Insulin pumps help persons with diabetes attain tighter glycemic control. We have relationships with four insulin pump manufacturers and suppliers: Accu-check, Animous, Medtronic, and Omnipod.
